Children's Outreach Support Worker - Holystone, NE27 0JA

We are looking for a children’s outreach support worker for an 8 year old boy with a diagnosis of Ullrich congenital muscular dystrophy which means he has low muscle tone and hypermobility of joints and requires constant support. He is not weight bearing , so moving and handling are becoming more difficult. He depends on others for personal care, dressing, teeth cleaning, toileting and help in and out of his chair. He also needs help to change position in his chair and bed. He has a PEG feed every morning and night to support his target weight. He can eat small amounts of solid food, but needs to be cut small as he needs more strength in his hands to do this on his own
He has full-time one to one school support to help with his needs.
He needs support with accessing activities;/groups by an adult who is able to meet all of the above needs.

Personal Care
Provide direct assistance to a lady in North Shields to make her daily life more comfortable, enjoyable and meaningful in a pleasant way.
Help her around the house, getting out of bed, getting clothes on, getting showered/bathed.
Attitude
Attitude is key – you must work with her in an empathetic way so as not to make her feel anxious or controlled (eg, questioning her requests, not responding to her etc., standing behind her when she’s watching tele).
You should be ‘on call’ during work hours to assist her as required – this is the first priority. Think of her first, help her get around the house and prepare things that she needs (washing/helping with boots on/off etc.) Take her places as needed – out of house.
Medical Care
Organise the medical help she needs on a daily/weekly/monthly basis.
Coordinate and box up weekly meds.
Track stock of meds and re-order meds when needed – create a planner to track this. Liaise with the doctors and pharmacy on a regular basis.
Monitor and organise medical appointments.
Drive her to all of her medical appointments when needed (Doctors/Hospital).
Clean out the meds cupboard on a regular basis and dispose of old medication.
Nutrition
Organise daily food & drink requirements for the lady.
Make sure that she gets sufficient food and drinks throughout the working day
Prepare meals (Breakfast, Lunch and snacks).
Encourage her to have some food beyond just an Ensure drink.
Restock the food in the house from the local store on a regular basis.
House
Ensure the house is kept in a manner which helps the lady to enjoy being in the house.
Keep living areas clean and tidy (Bedroom/Kitchen/Lounge).
Hoover and Mop floors – weekly.
Dust all areas of the house – biweekly.
Empty ashtrays from outside – 2 x weekly.
Organise and tidy up all areas regularly (Bedside Table, Kitchen Bench).
Put washing in on a regular basis, hang up etc when finished.
Put dishwasher on and empty.
Change the lady’s bed sheets once a week.
Ensure her key things are easily accessible. (For eg, Vapes, Cigarettes, Headphones, Coat etc).
Ensure the dogs are taken care of (water / food / letting out the back).
Socialisation
Plan and organise trips out of the house for her when she is feeling up to it.
Meeting friends and family.
Shopping.
Getting out of house into nature (beach).
Hours – 10am to 3pm Monday to Friday
